Chronos Longevity Assessment and Therapy Inc.

1. Acceptance of Terms

By accessing or using the services offered by Chronos Longevity Assessment and Therapy Inc. (“Chronos Longevity,” “we,” “us,” or “our”), including our website, online forms, virtual or in-person assessments, or any related materials, you agree to be bound by these Terms of Use. If you do not agree with these terms, please do not use our services.

2. Scope of Services

Chronos Longevity provides private, data-driven health and lifestyle consultancy services focused on healthspan, longevity, and physical performance. Services may include virtual consultations, in-person assessments, lifestyle analysis, strength testing, mobility screening, and personalized coaching recommendations.

We do not provide medical care, diagnosis, treatment, or physiotherapy services. Participation in any service does not establish a patient–practitioner relationship, nor is it a substitute for care provided by a licensed medical or healthcare provider.

3. Eligibility

You must be at least 18 years of age to use our services unless a parent or legal guardian provides written consent. By booking or submitting any personal information, you represent that all information provided is accurate, current, and complete to the best of your knowledge.

4. Client Responsibilities

You agree to:

• • Provide truthful and complete information on all intake forms

• • Inform us of any relevant health conditions or physical limitations

• • Use your own judgment in applying any recommendations or strategies provided

• • Consult your physician before making significant health or lifestyle changes, especially if you have a medical condition or are on prescription medication

5. Payment Terms

All services are private-pay and not eligible for reimbursement through public health insurance plans or extended medical benefits.

Full payment is required at the time of booking unless otherwise arranged. Prices are listed in Canadian dollars (CAD) and are subject to applicable GST.

Credit card information is securely processed and stored through our third-party provider, Jane App. Clients may be required to maintain a valid credit card on file for booking and cancellation purposes.

6. Cancellation & Refund Policy

You may cancel or reschedule your appointment up to 24 hours in advance without penalty. Cancellations made with less than 24 hours’ notice may be subject to a cancellation fee equal to the full session price. No-shows are non-refundable.

Automated services, such as digital report purchases, are final sale and non-refundable once delivered.

7. Limitation of Liability

To the fullest extent permitted by law, Chronos Longevity and its representatives are not liable for any direct or indirect loss, injury, or damage arising from the use of our services or reliance on any information provided. You agree to use our services voluntarily and at your own risk.

8. Intellectual Property

All content provided by Chronos Longevity—including assessment frameworks, reports, digital materials, graphics, and written content—is the intellectual property of Chronos Longevity and may not be reproduced, distributed, or shared without express written permission.

9. Privacy

All personal and health-related information is collected and managed in accordance with our Privacy Policy. By using our services, you consent to the collection and use of your information as described therein.

10. Governing Law

These Terms of Use are governed by and construed in accordance with the laws of the Province of British Columbia and the federal laws of Canada applicable therein. Any disputes shall be handled in the courts of British Columbia.

11. Modifications

We reserve the right to update these Terms of Use at any time. Updated terms will be posted on our website with the new effective date. Continued use of our services after changes constitutes acceptance of the revised terms.
